You’ll receive a link to complete intake forms online before your first visit. This typically takes 15–20 minutes and covers your health history, current concerns, and goals for care.
Our team will reach out to confirm your appointment details and answer any questions you have before your first visit.
Arrive 10 minutes early for your first in-person visit, or log into the telehealth platform a few minutes before your virtual session. Your clinician will guide you through a thorough initial evaluation.
Following your evaluation, your provider will work with you to develop a personalized treatment plan tailored to your goals, lifestyle, and preferences.
All paperwork is completed securely online through our IntakeQ portal — no printing required. New patient bundles, release of information forms, assessments, and document uploads are all on our dedicated forms page.
